PLR Transcript Documentation Policy

Students wishing to receive credit for programs, courses, or credentials reviewed by the University through the Professional Learning Review (PLR) process generally submit documentation in one of two ways, either as a notarized credential or a transcript from the reviewed organization:

  1. For reviewed credentials like licenses and certifications, appropriate documentation is frequently a notarized copy of the license or certificate and a current renewal card (where appropriate). Additional documentation may be required. Any notarized documentation related to a PLR should be submitted along with the Student Submission of TESU-Assessed Credentials.
  2. For transcript documentation from the reviewed organization, you must request the organization include the following data elements required by the Office of the Registrar:
    • Student Name
    • Current address
    • Social Security Number, unique student/employee ID number or date of birth
    • Course title
    • Course number if applicable
    • Date of Completion
    • Credit Value
    • Passing grade
    • Organizational representative authorized signature
    • Organization’s seal/letterhead

All documents should be forwarded to:

Office of the Registrar
Thomas Edison State University
111 W. State St.
Trenton, NJ 08608

Documents are considered official only when sent directly from the source/record holder of the PLR documentation (i.e., the reviewed organization). Best practices suggest the following methods are used to ensure records are official:

  • Sent directly from source to Thomas Edison State University Office of the Registrar
  • Letterhead or typed return address on source envelope
  • Seal, label or signature adhered across back fold of source envelope
  • Postage applied with postage meter
  • Postmark from city and state of official record source

Any document received that could have been intercepted by or sent from anyone other than source/record holder will not be accepted as official.
